Cardiac Wellness Program

The Cardiac Wellness Program is a free education and exercise program for people with heart disease and those at risk of developing a heart related condition.

Who can benefit from this program?

Those who have had:

  • Angina, a heart attack, angioplasty or stent, bypass surgery or valve surgery
  • Implantable Cardiodefibrillator
  • Heart Failure
  • Had a heart transplant or currently waiting a transplant

Those with risk factors such as:

  • Diabetes
  • High cholesterol
  • High blood pressure
  • Obesity
  • Smoking
  • Increased stress levels and inactive lifestyles

Your Cardiac Wellness Team:

  • Physician
  • Nurse
  • Dietician
  • Physiotherapist
  • Pharmacist
  • Social Worker
  • Respiratory Therapist

What to expect of the program

Initial Assessment: To review medical history and check for risk factors prior to starting the program. You may need a stress test before the program begins.

Education Sessions: Offered weekly or twice a week on topics such as heart healthy eating, medications, quitting smoking, stress management, blood pressure and cholesterol.

Exercise Sessions: Weekly or biweekly one hour sessions. Includes circuit training, streng-thening and cardio exercises using treadmills, walking, cycling. Blood pressure and heart rate are monitored during the program.

Follow-ups: Offered at 3 and 6 months after finishing the program.

How do I get started?

See your doctor to be referred to the program. You will then receive a call for an initial assessment appointment. A family member or friend is welcome to join you in the program.
